HOSPICE BENEFIT COVERAGE

Four levels of care are covered by the hospice benefit: routine home care, continuous home care, inpatient respite care.

Note: Respite care is covered when necessary and is limited to no more than five days at a time. General Hospice patients may shift among the levels of care, depending on their needs, the needs of family members caring for them, and the determinations of the medical team managing their care.

HOSPICE CARE SETTINGS

Hospice care may be provided in a number of settings, e.g.,

your home, a Medicare-certified hospice facility, or an authorized inpatient acute care facility. Care can shift among settings without affecting the benefit or requiring additional authorization.

SERVICES WE PROVIDE

Routine Home Care

Rite Care Hospice provides residental care in patient’s homes and has established partnershops with long-term care facilities and nursing homes, assisted living facilities and residental care facilities for the elderly.

Dedicated Inpatient Units

If a patient needs care beyond what can be provided at home, we have special arrangements with local facilities where patients may receive necessary and appropriate care and return home as soon as possible.

Continuous Care

If a patient develops the need for more constant care, we can provide shifts of hospice staff at the home for a brief period so the patient can avoid being transferred to a hospital.

Medications, Medical Supplies

Medical Supplies, and Durable Medical Equipment related to the palliation and management of the terminal illness related conditions are provided.

Respite Care

Limited to five consecutive days, respite care provides a brief "respite" for the patient’s primary caregiver by admitting the home care patient to an institutional setting without meeting the "inpatient" pain and symptom management criteria.

Bereavement services

Are available for family members and other individuals for up to thirteen months following the death of a loved one.

CONTINUITY OF CARE

The Primary Physician

Upon admission to the hospice program, the patient’s primary care physician will be notifed of the patient’s acceptance to the program, and he will take a place on the interdisciplinary team.

If the patient’s primary care physician does not want to follow the patient any longer, or the patient would prefer  to have another physician, then the patient will be assigned to the hospice medical director. Medicare will make payment for services provided by a physician other than the hospice physician or designee. When the family arranges these services, hospice must be informed.

TESTIMONIALS

review-1.jpg

When my grandfather’s health was rapidly declining in front of our eyes a family friend referred Rite Care Hospice. The Rite Care staff always made sure that my grandfather had everything he needed, from medical equipment, prompt medication deliveries, supplies and even an around clock nurse when his condition changed for the worse. I can confidently say that my grandfather was not in any pain thanks to the dedicated team of Rite Care Hospice. When my grandfather passed away it was very hard on my family. The spiritual counselor from  Rite Care Hospice was one of the kindest people I have met. He provided Bereavement serivces for my mother well over a year after my grandfathers passing. Losing a family member is very difficult in life but it is extremely comforting to know that there are professionals that help you through every step of the way like the ones at Rite Care Hospice
